Trong tình huống này, "in the setting of" được sử dụng để dẫn yếu tố nền có liên quan đến chẩn đoán. Cụm từ này không mang hàm ý "đổ lỗi", mà người viết hay người nói ở đây chỉ đang "nghi ngờ mối liên quan" giữa yếu tố nền và chẩn đoán.
