When I was at the Italian Cataract & Refractive surgery meeting ( Associazione Italiana di Chirurgia della Cataratta e Refrattiva ), I had the pleasure of meeting Dr Miguel Rechichi and I enjoyed the conversation so much that I invited him to be on our podcast to share his experiences with everyone. I attend many ophthalmic surgical lectures every year and many meetings in many countries. And you know that the lecture is amazing when I start taking notes! Dr Beeran Meghpara is a cornea, refractive, cataract, and anterior segment surgeon and professor at Wills Eye Hospital in Philadelphia, USA. His surgical pearls about nanophthalmic eyes and Fuchs' dystrophy eyes have helped me perform better surgery for my patients. We cover some of those important topics and then we have a detailed conversation about the reality of private practice versus academia versus private equity owned practices. It is truly eye opening! Ok, I'm kidding, but he is truly remarkable and there is so much to learn from him. Plus he is the only ophthalmologist I know who can give me a run in bench press. We have been hearing about in-office (or in-clinic) surgery suites where we can perform intra-ocular surgery such as cataract surgery without having to go to an ASC (ambulatory surgery center). Decades ago we shifted from performing ocular surgery in hospitals to out-patient surgery centers and today that is where we perform most of our cases. Some surgeons are now predicting that we will make another move, from the ASC to an in-office/in-clinic surgery suite. Our podcast guest today is Dr Parag Majmudar who runs the Chicago Cornea practice and is a Professor at Rush University. Sometimes we are so involved in our own practices that we forget about the very important public health aspects of ophthalmology. This includes so much such as determining the best procedures and practices for our patients but also how to critically look at the data. When we see a Medicare cut to certain MIGS procedures, is it any surprise that the other MIGS procedures grow much faster in popularity among surgeons? We explore these topics and much more with our podcast guest Professor Kuldev Singh MD MPH who is a glaucoma specialist at Stanford University.
