Over time, we end up accumulating random questionnaires, surveys, and evaluations to include as part of our new patient intake process. And while each one provides another perspective on what might be going on, this isn’t the best experience for your patient who is already overwhelmed and in need of your help.

Having to think through and complete each intake form may feel ‘comprehensive’ to you but to the patient, it’s overkill. Not to mention the time it takes for you to review all of them and try to make sense of the information before seeing your patient for the first time.

Working on your intake forms might not be the most appealing thing to do – but trust me, when they include the right kind of questions, it makes your job so much easier and lets your patient’s know that you are serious about getting to the root of their issue.
